How much would it cost to start a small TV station?

I see that the courts just reinforced the "must carry" rule -- meaning, even the smallest of broadcast TV stations must be carried on cable networks. I wonder how one would start a small, broadcast TV station, therefore? I would take it a license is needed, but the typical TV can get 100 channels via antenna and only a few of those channels actually get reception.
